Mycotic aneurysms of coronary arteries.

چکیده
Bacterial endocarditis may give rise to mycotic arterial aneurysms in any part of the body, but particularly in the thoracic aorta, intracranial vessels, superior mesenteric artery, and the large arteries to the extremities (Weintraub and Abrams, I968). Aneurysms occurring in the coronary arteries are commonly arteriosclerotic or congenital and are very rarely mycotic in origin (Daoud et al., I963). This report concerns a patient in whom the diagnosis of mycotic coronary arterial aneurysms was made in life by means of coronary arteriography.

Mycotic aneurysms of the coronary arteries are extremely rare, with 15 cases reported. We report the successful treatment of a mycotic aneurysm of the left anterior descending coronary artery by coronary artery bypass grafting with aneurysm ligation and resection without the use of cardiopulmonary bypass.

Open surgery is the elective treatment for mycotic aneurysms of the aorta. This surgery consists of resection of the aneurysm, debridement and revascularization with an in situ or extra-anatomic bypass. Even when surgery has been successful, the morbi-mortality is raised and the endovascular treatment has become an alternative for specific patients. When mycotic aneurysms involved the visceral ...

In the case presented, multiple aneurysms of the peripheral pulmonary arteries occurred in a child with secondary pulmonary hypertension complicated by bacterial endocarditis. Study of other reported cases suggests that pulmonary hypertension may be significantly associated with the development of mycotic aneurysms. As surgical resection of aneurysms has been at least temporarily successful, in...
